The Trump administration's final drug pricing rule will save Medicare 96% less than the initial proposal, according to an analysis of the regulation published September 30 in Washington.

The final rule — the Global Benchmark for Efficient Drug Pricing, or GLOBE — targets Medicare prices for physician-administered drugs. Its scope has narrowed dramatically during the rulemaking process: the version that emerged applies to only four companies.

GLOBE is one of two mandatory pilot programs the administration designed to implement President Trump's most-favored-nation policy, which presses drugmakers to bring Medicare prices down to the levels charged in other wealthy countries. The second pilot, called GUARD, remains at the proposed-rule stage and has not yet been finalized.

For R&D and portfolio managers at affected manufacturers, the practical consequences are immediate but contained. Four companies now face mandatory benchmarking of their physician-administered drug prices against international reference prices, while the broader industry watches to see whether GUARD — which would extend the administration's leverage — advances through rulemaking or stalls.

The 96% reduction in projected savings between draft and final rule signals how much the administration conceded during the comment and review process. It also leaves open the question of whether the most-favored-nation framework, announced with expansive ambitions, will meaningfully shift drug pricing in Medicare or remain a narrow demonstration covering a handful of firms.

The administration has not yet finalized GUARD, and no date for its completion appears in the published rulemaking record.
